Approval without deliberation

What it looks like. "Approve" is whatever the approval surface accepts. Two hundred changes in one dialog. An approval queue reviewed at the end of a long shift. A request marked urgent that skips a step for good operational reasons.

Why it fails. The audit records N approvals; the human evaluated four. The authority is real and the deliberation behind it is not, so every downstream control that trusts "this was approved" is trusting something that did not happen.

Instead. Put validity predicates on approval events: human identity, context actually presented, decision freshness, ordering before the action, and a minimum review interval for high-consequence items. Reject events that fail the predicate rather than recording them as approvals. Urgency raises the verification bar; it never lowers it.
